🏈 Sports Physicals Required for All PWYFA Athletes 🩺

To ensure the health and safety of all participants, every PWYFA football player and cheerleader is required to have a completed sports physical on file before participating in the 2025 season.

📅 Deadline: August 1, 2025

All physical forms must be submitted to your child’s Team Mom by this date. No athlete will be permitted to practice or compete without a completed physical on file.

✅ Who Can Perform the Physical?

Your child’s primary care physician can perform the physical exam. Please make sure the required form is used and fully completed.

No appointment necessary – walk-ins welcome.

⚠️ Remember:

• Submit the completed form to your Team Mom by August 1st.
